The Voluntary LTD plan
provides monthly benefit payments for loss of income in the event
of a disability which results from Accident or Sickness.

Employees can choose
from $300 to $5000 per month. Not to exceed 60% income. |
Portable to age 65 at
the same rates after one year in force.
Guaranteed issue up to
$3000. No evidence of insurability required.
Pre-existing condition
exclusion is 12/12.
Pregnancy is covered
as any other illness.
Partial Disability Benefit
pays 50% of the benefit for three months.
Survivor Benefit pays
benefit for 3 months.
Issue age rated.
