Short Term Disability
Especially Designed for the Employees of
CHAMBER OF COMMERCE ASSOCIATION OF ALABAMA
The Voluntary STD plan provides monthly benefit payments for loss
of income in the event of a disability which results from non-occupational
Accident or Sickness.
WAITING PERIODS 15 Days Accident, 15 Days Sickness
BENEFIT PERIOD 6 months
BENEFIT AMOUNTS Employees can choose from $300 to $3000 per
month.
Not to exceed 65% of income.
- Portable after one year in force.
- Guaranteed Issue. No evidence of insurability required.
- Pre-existing condition exclusion is 12/12.
- Pre-existing condition benefit
pays 25% of benefit for six weeks for claims within first 12 months.
(Not applicable to replacement policies.)
- Pregnancy is covered as any other illness.
- Partial Disability Benefit
pays 50% of the benefit for three months.
- Survivor Benefit pays benefit for 3 months.
- Issue Age-Rated. (Rates based on age at time of policy issue)
